Did he ever actually do the meth? I thought he had cancer or something. I never watched the entire series though.

Not as far as I've seen in the series. But the first pic is taken from malcom in the middle anyways so the fact that he didn't do meth in breaking bad isn't really relevant to the joke.

No, the joke is just playing off how different his characters look in the different shows. Don't take it literally.
